By providing your personal information, you are consenting to 3M posting your information on its website(s) for patients and other individuals to search. As part of this program, 3M will only use the data as provided in these Terms of Use, and in accordance with the 3M privacy policy.

3. Access to or Removal of Personal Information. You may review, correct, update or remove the personal information that you provide to us by contacting 3M through the form below.

3M reserves the right to discontinue the Find-a-Doc Search engine program or a doctor's participation in the program without notice. Since this is an optional and free program, 3M disclaims all warranties and shall have no liability in connection with this program.
